19 yo FTM top. 3 months post operative. This was a double incision surgery with free nipple grafting. I do my best to get rid of dog ears. Insurance does not cover liposuction for this. In this case, I took the excision as far as the surgical field allowed. I don’t reposition patients onto their newly closed incisions to chase dog ears. Sometimes they will resolve on their own. If not, a simple in office procedure can take care of them. If upper pole fullness is not present before surgery, this will be apparent after the top surgery is performed. Pectoralis implants or a second surgery with fat grafting can help correct this.
